Our role in GUIDE
As your local partner in Metro Atlanta, Visiting Nurse private Home Care helps make GUIDE practical and accessible for families by providing hands-on, community-based support.
We help eligible caregivers schedule annual respite hours within our service area, making it easier to take a break when they need it most.
We coordinate in-home support and stay in close communication with PocketRN so each family’s care plan remains aligned and up to date.
We work alongside PocketRN’s Nurse For Life model, ensuring families have a dedicated nurse who knows their story, checks in regularly, and coordinates with physicians, specialists, and local community resources.
Our goal is simple: to make dementia care feel less fragmented and more supportive, consistent, and human.

When you call, it’s helpful to have your Medicare card, any Power of Attorney (POA) paperwork, a current medication list, and any dementia diagnosis documentation (if available). If a diagnosis hasn’t been formally documented yet, PocketRN can help guide you through the next steps.
